When you send us a message through a contact form, we receive your name, email address, phone number, how you would like us to reply, the best time to reach you and the message you write. Please don't put medical details in a message. Health history goes on the forms in our own secure portal, which opens from the New patients page.

How we use it

We use your message to reply to you and to schedule your visit. We do not sell your personal information.

Your health records

Your health records are kept in our practice records, not on this website. We keep them private and share them only as the law allows. Our Notice of Privacy Practices, available at the office, says how.

Cookies and tracking

This website sets no advertising or tracking cookies and uses no analytics, ad pixels or session recording. It uses only what is needed to load and display the pages.

Your choices

You can ask us what we hold about you, ask us to correct it, or ask us to stop contacting you. Call or write to us and we will reply.
